Hepatic portal venous gas from perforated sigmoid diverticulitis.

چکیده
A 68-year-old man was seen in the emergency department with fever, rigors and left lower quadrant pain 2 days after being discharged from hospital for treatment of sigmoid diverticulitis. He was febrile (body temperature 40°C) and hypotensive (blood pressure 90/60 mm Hg). There was a firm, tender mass in the left lower quadrant. His leukocyte count was 19.7 × 109/L. Urgent CT revealed the unusual finding of gas scattered throughout the portal venous branches in the liver (Fig. 1). There was air in an abscess in the left retroperitoneum, and gas was seen in the inferior mesenteric vein (Fig. 2). A diagnosis of a retroperitoneal abscess and portal pyelophlebitis was made, secondary to perforated sigmoid diverticulitis. Emergency laparotomy revealed a large diverticular mass in the left lower quadrant and an abscess extending from the sigmoid mesentery into the left retroperitoneum. The abscess was evacuated and a Hartmann’s resection was performed. Blood cultures grew Enterococcus and Pseudomonas spp. The patient had an uncomplicated postoperative course, and the colostomy was closed 6 months later. Discussion

Diverticulitis as a cause of septic thrombophlebitis: A literature review
Introduction: Perforated diverticulitis, with fistula formation between the colon and inferior mesenteric vein, is a cause of portal venous gas. In contrast to more common causes of portal venous air, such as mesenteric ischemia and gangrene, the prognosis is much better for patients with perforated diverticulitis. However, the diagnosis can be difficult to make, given the wide range of nonspec...
متن کاملDiverticulitis complicated by pylephlebitis: a case report
INTRODUCTION Pylephlebitis is defined as septic thrombophlebitis of the portal venous system, usually secondary to infection or inflammation in the abdomen. In the current report, we present a case of pylephlebitis that complicated the course of a very common pathology, diverticulitis. CASE PRESENTATION A 62-year-old Caucasian woman with a history of sigmoid diverticulitis presented to our fa...
متن کاملHepato-bronchial fistula secondary to perforated sigmoid diverticulitis: a case report
BACKGROUND Patients with diverticulitis are predisposed to hepatic abscesses via seeding through the portal circulation. Hepatic abscesses are well-documented sequelae of diverticulitis, however instances of progression to hepato-bronchial fistulization are rare. We present a case of diverticulitis associated with hepatic abscess leading to hepato-bronchial fistulization, which represents a nov...

Pylephlebitis is defined as septic thrombophlebitis of the portal vein and its branches. Although rare, portal mesenteric venous thrombosis and pylephlebit is can remain as potent ial ly l i fe threatening sequelae of ruptured diverticulitis. Treatment recommendations from recent reports have included urgent exploratory laparotomy, prolonged intravenous antibiotic therapy, and anticoagulation f...
متن کاملDiverticular Pylephlebitis and Polymicrobial Septicemia
Diverticulitis primarily affects the sigmoid colon and is often complicated by intra-abdominal abscesses and fistulas. Rarely, however, mesenteric venous thrombosis has been known to occur. Optimal management is still unclear. We report the first case of polymicrobial sepsis resulting from diverticular pylephlebitis, managed successfully with bowel rest, antibiotics, and anticoagulation.
